What a Clicking or Grinding Knee Actually Means

Most knee clicking is normal — but pain changes everything

That clicking, popping, or grinding from your knee is almost certainly not the sound of damage. Research pooling data from more than 36,000 people found that knee crepitus — the clinical term for any audible or felt noise the joint produces during movement — is present in roughly 41% of the general population, and in 36% of individuals whose knees are entirely pain-free. The NHS classifies painless clicking as a normal variant, not a warning sign in its own right.

The single question that changes the picture is whether the sound arrives with something else. Pain, locking, or giving way are the accompanying features that shift clicking from background noise into a signal worth investigating properly. Painless clicking alone, however frequent or loud, does not reliably indicate joint damage.

This distinction — sound alone versus sound alongside symptoms — is the framework everything else here builds on. A grinding sensation that accompanies persistent aching, or a click that precedes the knee buckling underfoot, deserves a different conversation to the occasional pop felt during a morning walk. Where the sound actually comes from

Three mechanical stories sit behind the different sounds a knee can make, and they occupy very different clinical categories.

The sharp, single pop that occurs when you straighten or squat is commonly explained by cavitation — a gas bubble collapsing within the synovial fluid that lubricates the joint. Think of it as the click of a knuckle: a pressure change, not a structural event. Clinicians widely offer this as the working explanation for painless, intermittent popping, though it is worth noting that the precise mechanism has not been fully resolved in the research literature — present it as the most plausible account rather than settled anatomy.

A snapping or clunking sensation — often felt rather than heard — tends to come from a tendon or ligament briefly catching on a bony prominence as the knee moves through its range. Like an elastic band flicking over a corner, it resolves the moment the structure clears and is frequently painless.

The third pattern is the one that sits in a different clinical category: a rough, sustained grinding or grating that persists across the arc of movement. This is more consistent with two imperfect surfaces — cartilage layers that have lost their smooth interface — moving against each other. Its significance depends heavily on whether pain accompanies it.

When clicking or grinding signals joint damage

Noise alongside pain, locking, or giving way sits in a different clinical category — and the data give some sense of how different.

A 2024 systematic review and meta-analysis of 103 studies covering 36,439 participants found that knee crepitus is associated with more than a threefold increased odds of a radiographic osteoarthritis diagnosis (OR 3.79, 95% CI 1.99–7.24). Crepitus also correlated with several OA-related features on MRI, suggesting the sound may reflect joint changes that an X-ray has yet to capture. These are meaningful associations — but not a diagnosis. Between-study heterogeneity was extreme (I² up to 98%), and evidence certainty across most included studies was rated low to very low. The odds ratio points in a direction; it does not tell any individual patient what their knee contains.

What makes these figures easier to interpret is the severity gradient they sit within. Crepitus is present in roughly 35% of people with ligament injuries, 61% of those with cartilage pathology, and 81% of those with established osteoarthritis. The same quality of sound carries a different clinical weight depending on the rest of the clinical picture. A grinding knee in an active 28-year-old following a ligament injury belongs to a different conversation to the same sound in someone whose knee has been stiff and aching for five years.

Functional changes are the features that most clearly shift clicking toward needing assessment: the knee failing to straighten fully (locking), buckling unexpectedly underfoot (giving way), or persistent swelling after activity. Any of these alongside noise — rather than noise alone — is the pattern that warrants a proper examination rather than watchful waiting.

Clicking at the kneecap — what active adults need to know

Clicking that concentrates at the front of the knee — on stairs, during squats, or after sitting for long periods with a bent knee — has a name: patellofemoral pain syndrome (PFPS). The condition arises where the kneecap (patella) tracks against the thigh bone (femur), and it accounts for a meaningful share of anterior knee clicking in physically active adults. The pattern that clinicians call the 'cinema sign' — discomfort that builds during a long film and eases when you stand — is a characteristic feature.

A 2024 cross-sectional study added a clinically useful detail: the severity of crepitus in PFPS correlates with lower quadriceps muscle bulk — specifically the rectus femoris and the vastus medialis, the inner quad that sits just above and to the inside of the kneecap. The association was with muscle mass rather than isometric strength, meaning it is bulk, not raw force production, that appears to matter here.

The mechanism follows logically. Reduced quadriceps bulk means less dynamic cushioning around the patella during movement, which increases surface contact and friction as the kneecap tracks its groove. Crepitus in this context works as a two-way signal: weaker quads generate more noise, and persistent noise may indicate a functional deficit.

This matters because it points towards a correctable problem. Progressive quadriceps rehabilitation — with particular attention to strengthening the vastus medialis — is a well-established first-line response. It also explains why clicking around the kneecap often worsens after a period of reduced activity: deconditioning removes the very cushion the joint relies on.

Post-injury knees — why younger patients should not be reassured too quickly

Age is not a reliable guide to how seriously post-injury knee noise should be taken. A 2025 study confirmed that crepitus following traumatic knee injury — including ACL rupture — is associated with the presence and worsening of structural OA-related features on imaging. That finding matters most for the younger patient who has been told their clicking is nothing to worry about simply because they are in their twenties or thirties.

There is a further complication: people who have injured their knee tend to underestimate their own crepitus considerably. In a post-ACL-reconstruction cohort, 62% of participants had crepitus confirmed on physical examination, while only 39% reported noticing it themselves. The self-report tool used in many clinical questionnaires (the KOOS-S2 item) achieves 73% specificity but just 47% sensitivity — which means it can indicate crepitus is present when it is, but it frequently misses cases that a hands-on examination would find.

For any patient who has had a significant knee injury and is now aware of new grinding or clicking, this is the practical take: an online symptom check or a telephone triage is unlikely to capture the full picture. Physical examination by a clinician — not a questionnaire alone — is the appropriate next step.

What a proper assessment of a noisy knee actually involves

The starting point for any useful assessment is a detailed clinical history: when exactly the noise occurs, whether it is reliably painful, and whether the knee has ever locked, given way, or swelled after activity. A history of previous injury — even one that felt minor at the time — changes the interpretation considerably, as the sections above set out. Hands-on joint examination follows, because questionnaires and self-report miss a substantial share of crepitus that a clinician would find on physical assessment.

Imaging is guided by what the history and examination suggest, not requested routinely. A plain X-ray identifies joint-space narrowing and bony change. MRI is more sensitive for cartilage integrity, ligament status, and soft-tissue detail — and because osteoarthritis now has an evidence base as a whole-joint disease involving cartilage, bone, synovium, and the surrounding musculature, a thorough picture may include all of those structures rather than a single focus area.

If structural damage is found, the treatment direction depends on the defect pattern, its severity, and individual patient factors. That range runs from targeted rehabilitation and load management through to injectable biologics and regenerative approaches. Suitability is a clinical judgement, not a symptom-list conclusion.
