Clicking, grinding, and catching are a different order of signal altogether. Age adds a further layer: patients in their teens to thirties more commonly sustain acute meniscal tears or focal articular defects from a single twisting event on the sports field, while those in their forties and beyond more often present with gradual articular thinning through wear. The symptoms in both groups can appear nearly identical, which is part of why imaging and specialist assessment carry more weight than age or mechanism alone.
What a proper assessment involves
A specialist assessment starts with three questions rather than a treatment list: which cartilage structure is involved, how extensive the damage is, and whether active intervention or structured conservative management is the right response. Those answers — not the level of pain on a given day — determine what happens next.
Reaching them typically involves a clinical history, a physical examination (joint-line tenderness, range of movement, and stability tests under load), and an MRI to grade cartilage involvement and confirm whether the damage is articular, meniscal, or both.
