What the swelling is actually telling you
Noticing that your knee swells up after a run or a gym session is unsettling — and it is worth taking seriously. That said, swelling alone does not confirm cartilage damage. It is your joint's way of flagging that something has irritated it; working out exactly what requires more information than the swelling itself can give you.
The mechanism is straightforward: when tissue inside the knee is irritated or injured, the synovial lining produces extra fluid to protect the joint. That fluid builds up in the joint capsule, which can hold 30–50 mL before any visible puffiness appears on the outside — so early changes can be happening internally before you notice anything obvious.
Several structures can trigger that response, from minor overuse to a meniscus tear to articular cartilage damage. They do not all look the same, and they do not all need the same management.
The dividing line between normal post-exercise soreness and something that deserves professional attention is broadly this: swelling that returns after every session, or that takes longer than 48–72 hours to settle, is a signal worth investigating rather than resting through.
How cartilage damage produces swelling
Articular cartilage — the smooth white tissue capping the ends of your knee bones — has no blood supply of its own. That single fact shapes how cartilage damage behaves over time.
When cartilage is worn or cracked, loose fragments can rub against the synovial lining, the membrane that lines the joint. The synovium responds by producing more fluid. The critical difference from simple overuse is that cartilage cannot draw on a blood supply to repair itself, so the source of irritation persists. Other causes that look similar
Several other conditions produce post-exercise knee swelling that can feel identical to cartilage damage from the outside — which is precisely why no one should self-diagnose on the basis of swelling alone.
Overuse and repetitive strain tend to cause mild puffiness that appears a few hours after activity and clears fully with a day's rest. There is usually no structural damage.
Meniscus tears produce swelling that builds slowly, typically peaking at 24–48 hours after the session. Clicking, a sense of locking, or the knee suddenly giving way are characteristic companions — features that differ from the load-dependent pattern of articular cartilage injury described in the previous section.
ACL tears announce themselves more dramatically: a large effusion develops within 2–4 hours of a specific injury event, almost always alongside an audible or felt 'pop' and immediate instability.
Bursitis creates localised warmth and surface puffiness around the kneecap rather than fluid inside the joint itself — a surface-level problem, not an intra-articular one.
Osteoarthritis drives synovitis-related swelling that worsens progressively with activity over months or years rather than appearing after a single session.
The honest clinical reality is that no pattern of timing or accompanying symptoms is specific enough to distinguish these reliably without imaging. That is not a limitation of this article — it is the reason MRI exists. Swelling is a signal to investigate, not a diagnosis.
Red flags that mean see a specialist now
Some presentations genuinely cannot wait for a GP appointment in a fortnight. If any of the following apply, seek specialist review promptly — not because every case is serious, but because delayed imaging of structural damage can allow it to progress.

How cartilage damage is diagnosed and graded

MRI is the investigation of choice for articular cartilage. Standard X-ray shows bone clearly but misses early cartilage lesions entirely; soft-tissue detail requires magnetic resonance imaging. When a consultant reviews the scan, damage is graded on a four-point scale: Grade 1 indicates signal change with the surface still intact; Grade 2 shows partial-depth lesions reaching less than half the cartilage thickness; Grade 3 describes deeper loss approaching or reaching the subchondral bone; Grade 4 denotes full-thickness loss with bone exposed. Lesion area matters as much as depth — a threshold of roughly 2–4 cm² separates cases suited to conservative or minimally invasive management from those where structural repair or transplantation warrants consideration.
Two 2025 studies drawing on the Osteoarthritis Initiative cohort (204 and 233 knees respectively) made a clinically important point: cartilage extent alone accounted for less symptom variance than factors such as patellar tendinitis, bone marrow lesions, and body mass index. How much a knee swells or hurts does not reliably indicate how severe the underlying cartilage lesion is — a finding that underlines why imaging, not symptom severity, drives treatment decisions.
A thorough specialist review goes beyond the scan. Assessing movement patterns and load distribution — how the knee behaves under the activities that provoke swelling — adds diagnostic context that MRI alone cannot supply, and directly shapes which treatment pathway is appropriate.
What a specialist assessment leads to
Immediate care follows the same sensible logic: rest, ice for fifteen to twenty minutes every two to three hours, compression, and elevation above heart level. Where swelling is mild and clears fully with rest, that may be sufficient. But when it recurs after every session, comes with mechanical symptoms, or has not settled within 48–72 hours, specialist assessment — not more waiting — is the appropriate next step.
Treatment tier is determined by what imaging shows, not by symptom severity alone. For low-grade lesions (Grade 1–2), structured rehabilitation and load management are typically the first steps, with monitoring to track any progression over time.
