Why getting the answer right matters

Knee pain is rarely simple to interpret — and when the question is whether you have cartilage damage or arthritis, the confusion is understandable. Both conditions hurt, both can cause swelling, and both can feel worse on stairs or after exercise. Yet the treatment paths lead in opposite directions.

A 35-year-old with a focal cartilage defect is a candidate for joint-preserving repair. The same person wrongly managed as an arthritis patient might wait months for conservative care that does nothing for their actual injury — or, in the worst case, be directed towards procedures designed for an entirely different problem.

Symptoms alone have real limits: the overlap between these two conditions is greater than most patients expect. The sections below walk through the clinical markers that help distinguish them, but a reliable answer requires imaging — MRI in particular — not self-assessment.

Symptoms that suggest cartilage damage

Several features tend to cluster together when the problem is focal cartilage damage rather than a degenerative condition.

Onset is usually tied to a specific moment. A twist, a collision, a fall, or a heavy landing — patients with cartilage injuries can often describe the exact event that started things. Pain is sharp and well-localised, meaning you can press a finger to the precise spot rather than gesturing vaguely at the whole joint.

Swelling arrives quickly. Significant joint swelling within a few hours of the injury is a common finding with cartilage and soft-tissue damage.

Mechanical signs are the most telling feature. 'Mechanical signs' simply means the knee is behaving abnormally during movement — a displaced fragment or torn piece of cartilage can catch on surrounding structures or temporarily block the joint. Specific symptoms include:

  • A catching or clicking sensation mid-movement
  • The knee locking — losing the ability to fully straighten
  • An audible or felt pop
  • A giving-way sensation, as though the joint buckles unexpectedly

Not all cartilage damage follows a single injury. Chondromalacia patella — softening of the cartilage on the underside of the kneecap — builds gradually, producing anterior knee pain that is worst when bending, kneeling, or climbing stairs. There may be no memorable moment of injury at all.

Symptoms that suggest knee arthritis

Arthritis signals itself differently — through pattern rather than event.

There is no clear starting moment. Patients often struggle to say when the pain began. It crept in over months or years, gradually narrowing how far they could walk or how long they could stand. Pain is typically a deep, diffuse ache spread across the whole joint, not a spot you can pinpoint with one finger.

Morning stiffness is a named marker. With osteoarthritis, the joint feels stiff and reluctant on waking, but loosens within roughly 15 to 30 minutes of gentle movement. Inflammatory arthritis — rheumatoid arthritis being the most common — tends to produce prolonged morning stiffness lasting an hour or more, often accompanied by swelling in both knees or other joints simultaneously.

Crepitus is generalised, not discrete. The grinding or crunching that accompanies arthritic movement is a broad sensation throughout the joint as surfaces that have lost their smooth cartilage layer move against each other. This differs from the specific catching or locking of a loose fragment described in the previous section.

Timing within the day matters. Arthritic pain commonly builds after sustained activity — a long walk, an afternoon on your feet — and tends to peak later in the day. Some patients also notice flares after cold or wet weather, though the mechanism for this is not fully established.

When they overlap — and why that complicates things

The causal chain runs in both directions — and that is what makes this question genuinely difficult.

Arthritis is itself one of the leading causes of cartilage damage: osteoarthritis wears the joint surface down through cumulative mechanical stress, while inflammatory arthritis such as rheumatoid disease destroys both cartilage and bone through immune-driven inflammation. At the same time, an untreated focal cartilage defect — particularly one larger than approximately 1 cm — is known to accelerate OA development. Damage leads to arthritis; arthritis leads to further damage.

For patients in mid-life, this overlap is common rather than exceptional. With a lifetime risk of symptomatic knee osteoarthritis estimated at around 40% in men and 47% in women, many people presenting with a focal cartilage injury already have some degree of early degenerative change in the same joint.

Once damage becomes widespread rather than localised, the clinical picture blurs further. The sharp, mechanical symptoms that distinguish focal injury can give way to a more diffuse ache resembling generalised arthritis — not because the original injury has resolved, but because the surrounding joint has changed around it.

A final complication compounds this: imaging findings and symptom severity frequently do not correspond. Mild structural changes can produce severe pain in one patient; substantial joint change can cause relatively little in another. Which condition is dominant — and which is actually driving the symptoms — is rarely something a symptom history alone can determine with confidence.

How specialists tell them apart

The diagnostic pathway follows a logical sequence, moving from the simplest tools to the most invasive only when genuinely needed.

Physical examination is the starting point. A specialist assesses where exactly tenderness sits — localised joint-line tenderness tends to indicate a focal or meniscal problem, whereas diffuse tenderness across the whole knee leans more towards degenerative change. Range of motion, joint stability, and specific manoeuvres complete the picture: the McMurray test, for instance, can reproduce symptoms from a meniscal tear and help separate a mechanical problem from a generalised one.

X-rays are often the first scan ordered, but show bone rather than cartilage. They can confirm joint space narrowing, bony spurs, and the structural changes that mark moderate-to-advanced arthritis — but a focal cartilage defect or early degeneration may be entirely invisible, so a normal X-ray does not rule out significant soft-tissue damage.

MRI is the gold standard for cartilage assessment, showing the joint surface in detail — including the location, depth, and extent of any defect, graded under the ICRS (International Cartilage Repair Society) classification. Advanced sequences such as T2 mapping and dGEMRIC can detect early compositional changes before any structural loss is visible on standard imaging, though these are not available at every centre. At specialist centres, AI-assisted MRI analysis (onMRI™) and objective biomechanical assessment via markerless motion capture (MAI Motion®) can add a further layer of diagnostic precision.

Arthroscopy is reserved for cases where imaging leaves genuine uncertainty — it allows direct visualisation of the joint surface and, if needed, treatment in the same procedure.

What the diagnosis actually changes

The diagnosis matters because the two conditions call for fundamentally different treatment paths — and following the wrong one can cost time the joint may not have.

For focal cartilage damage, particularly in younger or more active patients, the priority is joint preservation.
