ACL injury is the tearing of the anterior cruciate ligament (ACL) which is one of the major ligaments in your knee. ACL injuries occur during sports that involve sudden changes in direction and jumping like football, basketball and skiing. Patients usually feel a "pop" in the knee when an ACL injury happen. The knee may swell, feel unstable and become too painful to bear weight.

Speedy diagnosis and treatment plans starting immediately after ACL injury can significantly affect the outcome, therefore it is important to seek a doctor have appropriate knowledge and expertise with the ACL. Mesenchymal Stromal Cell (MSC) therapy is offered by MSK doctors for some patients to treat ACL injuries if pick up early.

ACL Ligaments are strong bands of tissue that connect your thighbone to your shinbone and helps stabilise your knee joint. Especially important when you walk on uneven ground or change direction. Most ACL injuries happen during sports and fitness activities that can put stress on the knee:

  • Suddenly slowing down
  • Cutting (changing direction)
  • Pivoting with your foot firmly planted - football
  • Landing from a jump incorrectly - netball
  • Stopping suddenly

TREATMENT OPTIONS

Depending on the severity of the ACL injury the thighbone could have partially dislocated within the knee joint and cause cartilage damage.  ACL injury treatment may include rest, bracing, rehabilitation exercises to help you regain strength and stability. Cell based repair with injection or surgical repair may be possible in some cases, but is only offered by experts in the ACL treatments.  Surgery to replace the torn ligament followed by rehabilitation is more common.

ACL RECONSTRUCTION

There are many surgical techniques for ACL reconstructions each have their pros and cons. Some would claim the most common technique is the best, which is not always true. It is important to understand the logic behind ACL reconstruction and make your choice appropriately. Selecting the surgeon is the most important task in the treatment process.

All in side ACL surgery is a minimally invasive tendon preserving ACL reconstruction technique offered by MSK doctors (Prof. Paul Lee). With advanced fellowship surgical training with this innovative technique, Prof Lee has further refine and develop this technique, publishing the results and advanced technique articles on the all-in-side ACL reconstruction. He treats professional and amateur athletes regularly. Combining the 'all-in-side’ and 'non muscle crushing' surgical technique, Prof. Lee provides the best biological platform for his patients to recover and achieve the best results. more on 'non muscle crushing surgery'

The advantages of the all-inside ACL reconstruction by MSK doctors:

  • less painful
  • bone preserving
  • single tendon harvest
  • predictably anatomic ACL reconstruction
  • most cosmetic ACL surgery available
  • day case procedure
  • non muscle crushing
  • Return to play at 12 week

EARLY MOBILITY AND REHABILIATION

The less traumatic surgical approach, superior stiffness of the graft and it's class-leading fixation allow for patients to be up and walking within a few hours of surgery. Reduced pain compared to other ACL techniques means that muscles can activate better and sooner without pain-inhibition. The high stiffness / low tension short ACL graft means it can be implanted with the knee fully straight. Full extension is the major factor that contributes to early thigh muscle activation and thus exercises can begin immediately post-op. With a stronger ACL graft held in place with the strongest fixation available, this has led to the MSK doctors accelerated rehabilitation program as we are confident in the strength. Return to contact sport can be as early as 3 months provided the correct return to sport criteria are met.
